
12 Week PrograM
Orientation
During the Orientation Phase, participants fill out an Assessment Questionnaire, and watch a short video that takes them through the initial coaching process, as well as introduce them to the Wellness Wheel.
This process helps them to reflect on their current state of health and well-being, as well as providing their personal coach insight into their goals and aspirations.
Weeks 1-4
4 x 50-MINUTE foundation coaching sessions
During the Foundation Phase, clients will meet with their personal coach to explore in-depth their Health and Wellness journey. During these sessions, you will be invited to reflect on your current state of health and wellbeing, explore your vision for your desired future, gain an understanding of your personal strengths and values, identify opportunities for growth, and co-create a care plan with your coach. The care plan will reflect your long-term goals and identify additional resources to support you on your journey. As part of this plan, your coach will help pair you with other Kindness Project practitioners to give you additional practical guidance dependent on your primary needs. You will meet with your coach every week for 50 minutes as you move through Phase 1.
In Phase 1, you can expect:
Self-Assessment of Current State of Health and Wellbeing
Explore and Establish Vision, Values, and Strengths
Identification of Areas of Discrepancy
Orientation and Mapping of Long-Term Goals
Identification of Potential Resources (including Kindness Project practitioners)
Establishment of Care Plan
Weeks 5-11
In the second phase of the program, you will continue to work with a coach who will assist you in creating and achieving practical and insightful goals that move you towards your vision. Part of this process will include helping you to identify any potential challenges you might face and strategies to overcome them. Your coach will help you integrate and implement any suggestions, recommendations, and Care Plan additions from other Kindness Project practitioners. You will meet with your coach every other week for (3) 25-minute sessions as you move through Phase 2.
During Phase 2, you will also have the opportunity to connect with specialist practitioners that will give you personalized and informed guidance, therapeutic intervention, and compassionate support that will magnify your success. Depending on your care plan, you will have the option to meet with your specialist(s) 2-4 times for up to 50 minutes.
In Phase 2 you can expect:
Brief Action Planning for Ongoing Behavior Change
Establishment and Revision of Action Steps
Identify area of Focus for the session
Explore possible goal(s) to work on between sessions
Explore possible barriers and collaborate on potential solutions
Discuss preferences for self-monitoring and accountability
Identify Resources (practitioners, education, etc) to support goal
Explore Confidence, Importance, and Feasibility of the goal(s) and revise to reflect findings
Subsequent Sessions Review and Revise previous goal(s) and add in new goals as appropriate
Specialized Sessions with the Practitioners best suited to support you on your journey
Week 12
During the last phase, you will have a final session with your coach where you will co-create a longer term plan to take with you on the next phase of your journey. You will explore ideas like self-coaching, self-monitoring, accountability, and maintaining motivation. You will also be provided with materials to continue in your success. Finally, you will have an opportunity to look back on what you have learned with the Kindness Project and celebrate your growth and success! You will meet with your coach for one 50 minute session in Phase 3.

Review and Revise Previous Goal (s)
Co-create a Maintenance/Ongoing Change Plan
Provide Goal Setting Materials
Explore Accountability
Celebrate!
Explore learning and celebrate successes!
